Important appointment information

Booking Policy

New Patients:

Upon booking your first appointment, you will receive an appointment confirmation, a Welcome email and the ability to join the patient portal.

You wil also receive our digital paperwork via a pre check-in text, email, and the patient portal. This digital link includes new patient paperwork and consents that are required for medical treatment.

All of our paperwork is sent electronically and is completed digitially via a computer, tablet, or smart phone.

All paperwork MUST be completed prior to your appointment.  New patients cannot be seen if paperwork is not completed.

Established Patients:

Consents must be signed yearly for all patients.

Cancellation Policy

We aim to provide excellent service to our patients including prompt availability of our services and therefore you will be billed for any late (≥10 minutes), missed, or cancelled appointments without a 24 hours notice.

The following fees apply:

New Patients:

New patients who are late ≥10 minutes, fail to show, or late cancel less than 24 hours of their initial appointment will have the following fees assessed:

1st late cancellation/no show: $75

2nd late cancellation/no show: No further appointments will be scheduled

Established Patients:

Established patients who fail to show or late cancel less than 24 hours of their appointment will have the following fees assessed:

1st late cancellation/no show: $50

2nd late cancellation/no show: $75

3rd late cancellation/no show: Dismissal from practice

NB: Late cancellation/no show fees are due prior to scheduling the next office visit.

The practice reserves the right to require pre-payment of visits at time of scheduling for anyone who repetitively cancels their appointments or who late cancels or no shows an appointment.
